We ask that when calling to schedule your first appointment, you have your insurance card ready to better serve you! 


When coming into the office, you will  be asked for your insurance card and driver's license. You may also have additional forms to complete if your insurance company requires it. 


Following, you will have a one-on-one consultation with the Doctor. The Doctor will perform a thorough orthopedic, neurological and chiropractic exam. We encourage you to bring any recent x-rays, MRIs, or CT scans to this visit.

Once a diagnosis is established, the Doctor will determine if you would benefit from chiropractic care. There will be a treatment plan individualized to your condition which will cover the type of treatment, the length of treatment and recommended home care. Short-term goals may include reduction of pain and restoring motion. Long-term may include functional independence and wellness care.

Please note you will also be required to bring in any claim numbers, auto insurance information, med pay information or attorney name and phone number. 



What to bring to your first appointment: 


Please bring the following to your appointment:

  • Driver's License 
  • Insurance Cards
  • Any previous x-rays, MRI's or other diagnostic imaging and blood work
  • A list of your prescriptions
  • If you are a minor, you must have parental consent or a guardian present
  • We also encourage you to bring your spouse or family member to the visit



Please note: We do not sell extended treatment plans or require pre-payment packages.  Our doctors understand each patient's condition is different from another patient. Your course of treatment will depend on your condition, severity and your health goals.
